Urgently need Shamoly's ward commissioner's office address

I am living in Shamoly and I have to get a certificate from ward commissioner. I am not even sure the ward number of Shamoly. I can't find the commissioner's office address and no one could tell me surely. Moreover I am confused with ward councilor and commissioner. Someone told me both are the same.

Can anyone here tell me the correct ward number of Shamoly and the address of ward commissioner's office, please. It would be really helpful.
